परिषदीय विद्यालयों में सहमति पत्र हेतु निम्नलिखित संशोधित प्रारूप प्राप्त किया गया है। जो नीचे प्रदर्शित है-

उच्च प्राथमिक विद्यालयों में सहमति पत्र द्वारा अभिभावकों से उनके पल्यों को स्कूल आने से सम्बन्धित सहमति दर्ज करनी है, ताकि अभिभावकों को विश्वाश में लेकर शिक्षण किया का सके।
